Rescheduling or cancellations

Please call +1-510-545-3360 or message me below for a free 20-minute phone consultation and to schedule appointments. Online therapy sessions are payable by Zelle, Venmo, Ivy Pay, credit cards, Health Savings Account (HSA), or Flexible Spending Account (FSA). If you need to cancel a session, please provide at least 48-hours notice to avoid being charged the full session fee.

 

Insurance Reimbursements

If you are eligible for out-of-network benefits through your health insurance plan, Berkeley Holistic Therapy can provide a superbill which you can use to claim for reimbursements from your insurance plan. We can also provide receipts for services, if needed for HSAs or FSAs. 

Good Faith Estimate

Clients who don’t have insurance or are not using insurance have the right to receive a good faith estimate of the expected cost of your health care. Clients have the right to receive a written good faith estimate when scheduling a service or upon request. This currently does not include clients who are seeking reimbursements from a health insurance plan for out-of-network benefits. For more information, please refer to this notice. If you have any questions, please discuss this with your therapist.

The HIPAA Notice of Privacy Practices is available for download here. Current clients can also request a paper copy by contacting your therapist.
